No babies and want a US visa?

Me and wife married 7 years with no kids till now. we have tried medical thing in our poor country with no results. our financial and career status is very good. we are thinking about visiting a friend in the US and in the same time ask for a good doctor. Can we till the consular about our correct intentions so it can help us get the visa approval? noting that we are applying for a touristic visa? or we should only mention the touristic part to not rescue get a refusal? and if we cant tell him, and since we hate to lie, what is the kind of visa that qualify us to make such a visit to US to have a doctor visit? and what are the procedures? sorry for all those questions, but we truly need your help.
